to the loss of our friend and colleague Dr. Martha Post the practice is
currently suspending business and will not be open for patients indefinitely. We
recommend that you seek treatment with your regular doctor, an urgent treatment
office or one of the following Dermatology offices.
MEDICAL RECORD REQUESTS
must go through the proper procedures. You can find the Release of Medical
Information form HERE and mail it
to our office or fax it to 859-260-2948. Please make sure you fill out the form
completely.
Please note,
that the law allows us up to 30 DAYS to process records requests. Due to the
high volume we are dealing with, they will be dealt with on a first come, first
served basis.
Also note that patients over the age of 18 must sign their own release.
Parents no longer have access to their children’s files once they turn 18.
